under CASLS Information (the nice little boy with the red shirt picture). First click on CASLS forms and then the Member Plan. This form can be completed electronically then printed off for signatures. Please be sure to keep a copy of your report before getting all the required signatures just in case it gets misplaced in the process. The absolute best way to avoid the misplaced paperwork is to have a short meeting with at least the principal of your school to go over the report and have it signed at the meeting.
